To celebrate my birthday I’ll be having a week of celebrations… This is for the Saturday party. Please ensure every person fills in this form, especially for the Saturday party as this information will be passed on to the door staff and you may not get in!

Please note that the buffet is now ordered so if you apply to come now sadly there is no option for food.
There is plenty of on street parking. You will have to pay although it isn’t much after 6pm and they do take cards in the machines. There is disabled parking outside the club. The club is inside Sheffield CAZ so if you’re in a non Euro 6 van you will need to pay £10 to drive in the zone. Cars are exempt.
Entry is subject to donation to The Sheffield & Rotherham Wildlife Trust & Crossroads Care (collection boxes on bar).
